When you are fighting for insurance approval for Lipedema surgery, having objective data is everything. To help secure my own coverage, I used these functional mobility exams to clearly demonstrate the physical impact and limitations Lipedema caused in my daily life. Below, you can download a blank Lipedema Physical Evaluation template, as well as the Upper Extremity Functional Index (UEFI) and Lower Extremity Functional Scale (LEFS). I highly recommend bringing these to your physical therapist or doctor to fill out and include as crucial evidence in your insurance appeal or approval packet.
